Epirubicin (4′‑Epidoxorubicin, NSC 256942) is a topoisomerase II inhibitor with an IC₅₀ of 12 μM. It intercalates DNA and inhibits Top2, with its major metabolite epirubicinol having about one‑tenth the activity. Epirubicin is an anthracycline glycoside (epimer of doxorubicin at the 4′‑position) derived from Streptomyces peucetius. In clinical use, it is approved for breast cancer and gastric cancer, and in research it is used as a chemotherapeutic agent and as a tool to study topoisomerase II‑mediated cardiotoxicity.
